Heart to Heart Hospice

Who We Are

Heart to Heart Hospice (formerly A-One Hospice, Inc.) is a state licensed and CHAP accredited hospice agency that consists of health care professionals from around your community. We provide services where ever the patient calls home in several counties, including Genesee. We offer a team approach to provide care to the whole person - Body, Mind and Spirit.

We provide compassionate health care to patients and their families who are facing a life-limiting illness. Every individual under our care receives the attention of a Hospice Team which includes:

A Personal Physician (Dr.) who directs their medical care through the hospice team.

Registered Nurses (RNs) whom are on-call 24hrs a day/7 days a week. They will monitor the individual's physical condition and work with the physician to make sure the patient is a comfortable and free of pain as possible.

Social Workers (SW) are available to offer supprt, information and referrals to community services.

Chaplians (SC) offer a unique style of ministry by spiritual care to ALL faiths, traditions and cultures.

Home Health Aids are available to assist with personal care, such as bathing and dressing.

Volunteers offer companionship and brief peroids of rest for caregivers. They provide a steady source of comfort and strength to the individual and their loved ones during this stressful time.

Physical, Occupational and Speech Therapy, as well as nutritional conseling are also available.

Bereavement Services are provided by professional staff and trained volunteers. Realizing that greiving the loss of a loved one is natural part of healing, services may vary in intesity and length.

We recruit and train compassionate volunteers from the local community and place them with patients and families in need of companionship and caregiver respite.

Becoming a Hospice Volunteer can be one of the most selfless and rewarding things you'll ever do. There are many ways you can get involved and donate your time locally. You do not need any medical skills; our volunteers only offer companionship. For example: You can sit and listen, pray together, just talk, listen to music, read a book, write letters, play a game, share a common hobby, provide pet therapy, do some light housekeeping or provide relief to a
caregiver.  
 
We provide free professional training, the opportunity to affect a life and a chance to gain relationships that will change you forever!  Office help always needed.
